The following are brief descriptions of the forms that parents will be required to complete upon enrollment of their child at Little Charms Daycare.  Little Charms will provide these forms to you once the enrollment process has begun.

The Registration Form is used for enrollment purposes.  It contains information, such as parent/guardian work and home phone numbers,  emergency contact persons and their numbers, physician’s name and number, persons to whom the child will be released to from care, and general comments about the child’s interests, physical disabilities, and personality.

The Child Medical Information and Medical Emergency Form provides vital medical information about the child.

The Child Development Form is used for parents to list everything from language development to sleeping and toilet habits, and from the state of their child’s health to what their child’s typical day is like.  It is useful information to help understand the child so better care can be provided based on his/her individual needs.

Permission Slips include travel permission for out-of-home activities, permission to seek emergency medical care  in an emergency, and permission to administer medication to a child.

The Child Care Contract provides protection for both parents and myself, the caregiver.  It includes names, addresses and phone numbers of the parties involved; hours and days of care; payment amount and schedule; overtime pay; additional fees eg. diapers, out-of-home activities; holidays; sick child care; notice of termination of care; and trial period.
